Welcome to Foundation Hair Academy!

We are happy that you have chosen to study at Foundation Hair Academy. Training for a new career is challenging and we hope you will focus your energies on your studies during your time here.

Students must familiarise themselves with the contents of the Student Handbook.  The handbook is full of information about the Academy and the policies and rules you will be expected to follow when you are attending.  You will be asked to acknowledge that you have read and understand the contents of the Student Handbook.

School Closures and Holidays

Foundation Hair Academy follows the same calendar as SD71 and will be closed from March 18th until April 1st for Spring Break and for the following Statutory Holidays:

Foundation Hair Salon and Academy is located in downtown Courtenay and is easily accessible by bus. We encourage students to walk, bike, carpool or take public transit, if possible.

Administrative Offices are located on-site at the salon. 

Parking

Avoid parking near the building or in front of adjacent businesses. Please ask a staff member for a map of appropriate parking areas.

Lunch Room 

All students have access to a common break room area where they can eat, drink, and take breaks. We ask all students to help keep this common area clean. Food and beverages are allowed to be consumed only in the lunchroom area. 

Smoking

Please keep in mind that the salon, academy, and surrounding areas are smoke-free environments. Avoid smoking near the building or in front of adjacent businesses. This also applies to e-cigarettes/vaporizers. Please ask with staff in order to find out if there are approved areas in which you can smoke.

RECYCLING

We are a Green Circle Salon, meaning we make sure to repurpose and recover up to 95% of the salon resources that were once considered waste; materials such as hair, leftover hair colour, foils, colour tubes, aerosol cans, paper and plastics. We are committed to being a sustainability leader in the industry through our education, action, and accountability. We recycle the following materials and ask students to take part in our system: glass, aluminum cans, newspaper, office paper, plastic, and more. DRESS CODE 

Foundation Hair Academy encourages a fashion-forward, professional environment. Students are encouraged to express themselves creatively through personal style. Clothing should always be clean and hair/makeup should be appropriate for work.

  • All students are expected to maintain high standards of personal hygiene throughout their time at the academy. This includes proper grooming and cleanliness practices.

  • Students who come to work dressed inappropriately will be sent home to change.

  • All clothing must be work-appropriate. Clothing suitable for workouts, sports, or outdoor activities will be deemed inappropriate for business.

  • Open-toe shoes are not permitted on the salon floor due to the risk of injury.

  • Visible nipples or genitalia will be deemed inappropriate for work and you will be asked to change.

CODE OF CONDUCT

Expectations for Students:

  • Attend school in accordance with the Attendance Policy.

  • Treat all students and staff with respect.

  • Dress in accordance with the expectations of an employee in the industry relevant to your program of study; in a tidy, clean and fashion-forward manner.

  • Treat school property with respect.

  • Complete all assignments and examinations on the scheduled completion dates.

  • Avoid any conduct that may be deemed to be detrimental or damaging to the other students, staff members, or the institution.

  • Prohibited conduct under the Code includes but is not limited to:

  • any assaulting, disruptive, intimidating or offensive classroom behaviours to another individual; 

  • possessing or bringing weapons of any kind (i.e. knives, guns) to campus; 

  • using, possessing or distributing alcohol or drugs; 

  • making inappropriate remarks concerning another student or staff members’ ethnicity, race, religion or sexual orientation;

  • supplying false information or misusing any of the Academy’s records and/or information; 

  • making false accusations; disrupting classroom or campus lectures and activities.

  • any other conduct that is determined to be detrimental or damaging to other students, staff members or the Institution.

Any of the following, if substantiated, will result in immediate dismissal without a warning letter or probationary period:

  • Sexual assault to another individual or group;

  • Physical assault or any other violent acts committed on or off campus against any student or staff member;

  • Verbal abuse or threats;

  • Vandalism of school property.

  • Theft of the academy’s property, records or confidential information.

Students who do not meet the expected code of conduct will be subjected to the procedures in the Academy’s RESPECTFUL-FAIR TREATMENT POLICY, which may include immediate dismissal depending on the severity of the misconduct.

Use of cell phones and other technology

We encourage our students to use technology like phones, laptops or tablets in a professional way while in the program at Foundation. 

Personal cell phones on site must have ringers silenced before entering the salon. Cell phones are prohibited for use while working with clients, with the exception of taking photos or videos of the process. When not with clients, you can use your phone in a brief and professional capacity away from the salon floor, ensuring not to disturb other staff members.

SOCIAL MEDIA & TECHNOLOGY POLICY

Social media—personal use policy

  • The term 'social media' is used here to describe socially interactive networked information and communication by which personal information or opinions can be presented for public consumption on the Internet. Examples: Instagram, Facebook, LinkedIn, blogs, websites, text messaging and other social networking sites.

  • Students at Foundation Hair Academy are held to the following policies:

  • You must not use your website to attack or abuse staff, students or clients. You should respect the privacy and the feelings of others. You must not include contact details or pictures etc. of other students, members of staff or clients without their permission. If you break the law on your site (by posting something defamatory, for example), you will be personally responsible and may be detrimental to your studies or career.

  • If you think something on your site gives rise to concerns about a conflict of interest and in particular concerns about confidentiality, discuss it immediately with your Instructor or the Registrar/Director.

  • Don't reveal confidential information about Foundation Hair Academy or its staff, students, partner organizations or clients, including policy or details of either internal or private discussions. Consult with the Registrar/Director if you are unclear about what might be confidential.

  • If someone from the media/press contacts you about posts on your site, that relate to Foundation Salon and Academy, please discuss it with the Director before responding.

  • If you already have a personal social networking site or intend to create one, do not declare or imply that your content/views are representative of Foundation Hair Academy. If in any doubt, you may want to discuss your site content with the Registrar/Director. You may also want to include a simple and visible disclaimer such as "These are my personal views and not those of Foundation Salon and Academy”

Bring Your Own Device Policy

Students can bring their own devices (phones, tablets, laptops etc) to school for educational purposes. In the classroom, students will use devices responsibly and professionally, and only with the permission and direction of the Instructor or other staff member. Always ask clients and models for permission before taking photo or video content

Client Photo Consent Policy for Social Media Content Creation

Requesting Permission

  • Prior to taking any photos, the hairstylist student must politely ask the client for their consent to capture images of the finished hairstyle.

Explain Purpose

  • The student should briefly explain that the photos will be used for educational and promotional purposes on the student's professional social media accounts.

Provide Assurance

  • Assure the client that their personal information will not be disclosed, and the photos will be used in a respectful and professional manner.

ADMISSIONS POLICY

Foundation Hair Academy is committed to enrolling students who meet specific program admission criteria, including all academic standards and course requirements.  The Academy is dedicated to helping students succeed in achieving their education and career goals.

Procedure:

Once it is established the student has met all admission requirements, the Academic Programs Manager will:

  • prepare and review the enrolment contract, along with all relevant policies, with the student. 

Once the student has officially registered for the program (including a $500.00 deposit through our website) the student will receive the following:

  1. A Student Handbook containing:

  • Tuition and Fee Refund Policy;

  • Dispute Resolution

  • Grade Appeal Policy;

  • Withdrawal Policy;

  • Dismissal Policy;

  • Attendance Policy;

  • Use and Disclosure of Personal Information Policy;

  • Safety Policy.

ATTENDANCE POLICY

Foundation Hair Academy expects students to attend classes regularly and to be punctual while completing a program of study. This includes attendance to all labs, lectures, classroom activities, quizzes, tests and examinations. Students who are absent for more than 20% of a course or more than 30% of an overall program, may receive an incomplete grade for that course or program, respectively.

Procedure:

  1. Attendance hours will be recorded by each student enrolled in the program of study at the beginning and end of each day using the Attendance Sheet provided by the Academy.

  2. It is the responsibility of each student to contact their instructor in the event they will be absent or late in arriving to class. Reported absences will be marked as “excused”. Absences or late arrivals will be marked as “unexcused” for any student who does not advise the Academy of their intended absence or late arrival. In the case of any absence, whether excused or not, students will be responsible for making up the time lost, and all work, quizzes, tests, labs, and/or exams assigned during the student’s absence.

  3. Students may apply for a leave of absence in the case of an unavoidable personal or family emergency. Students must submit a Leave of Absence Request and deliver that in person, by email or by registered mail to the Academic Programs Manager prior to being granted a leave. Leaves of absence may not be granted for more than a period of two (2) weeks and must be substantiated by providing the Academy with all relevant documentation (including, but not limited to, a doctor’s note or report) to support their application for leave.   

  4. A leave of absence does not excuse the student from completing the work, quizzes, tests, labs, and exams assigned during the student’s leave. In the event a leave of absence constitutes an absence of more than 20% of a course or 30% of an overall program, the student may be required to repeat the course or program at their own expense.   

  5. Program end dates will not be extended due to absences or a leave of absence. Students are expected to complete their program of study within the program end date indicated on the Enrolment Contract.

  6. Students will not be granted a leave of absence for vacations, study for exams, or visits from family or friends.

  7. Whenever possible, the Academy will make a reasonable effort to accommodate an excused absence or approved leave of absence by re-scheduling the student into an alternate class for the same course on their return; or by providing the instructional materials missed during their absence; and/or rescheduling a time to write a missed test or exam; however, it is ultimately the responsibility of the student to make-up any instruction, tests or exams missed during their absence.

  8. Excessive absence or lateness may be a reason for dismissal from study.

TUITION REFUND POLICY

Eligibility for Refund:

Withdrawal from the Program:

- If a student decides to withdraw from the program, they may be eligible for a tuition refund under certain circumstances.

  • at least 30 days before the program start date, the institution may retain up to 10% of the tuition due under the student enrollment contract, to a maximum of $300.

  • less than 30 days before the program start date, the institution may retain up to 20% of the tuition due under the student enrollment contract, to a maximum of $600.00.

Notification Deadline:

- To be considered for a refund, the student must notify the academy in writing within 7 days of their decision to withdraw.

Tuition refunds after the program start date:

  • up to and including 10% of the hours of instruction to be provided during the program term have been provided, the institution may retain up to 30% of the tuition due to a maximum of $900.00

  • after more than 10% but before 30% of the hours of instruction to be provided during the program term have been provided, the institution may retain up to 50% of the tuition due to a maximum of $1500.00

  • after more than 30% of the hours of instruction to be provided during the program term have been provided, the institution may retain up to 75% of the tuition due to a maximum of $2250.00

Non-Refundable Fees:

The following fees are non-refundable and will not be included in any refund calculation:

Supplies and Materials Fee

  • Student kits and textbooks are non-refundable

Exceptional Circumstances:

In exceptional circumstances (e.g., documented medical reasons), the academy will consider refunds on a case-by-case basis.

Refund Process:

Submission of Request:

- The student must submit a written request for a refund to the academy's administration office.

Review and Approval:

- The request will be reviewed by the administration, and if eligible, the refund will be approved within 14 days of the received request.

Processing Time:

- The refund will be processed within 30 days of approval.

No Refund for Expelled Students:

Students who are dismissed from the program due to violations of academy policies are not eligible for a refund.

DISPUTE RESOLUTION POLICY

Foundation Hair Academy provides an opportunity for students to resolve disputes in a fair and equitable manner.

The policy applies to all students who are currently enrolled or were enrolled 30 days prior to submitting a notice of written concern(s) to the Academic Programs Manager.

Procedure for Student Disputes:

  1. When a concern arises, the student should address the concern with the individual most directly involved and the Instructor of the course or program the student is enrolled in.  If the student is not satisfied with the outcome at this level, the student should put his/her concern(s) in writing and deliver this written notice to the Academic Programs Manager, either in person, by email or by registered letter.

  2. The Academic Programs Manager will arrange to meet with the student to discuss the concern(s) and desired resolution(s) within 5 business days of receipt of the notice.  

  3. Following the meeting with the student, the Senior Educational Administrator and/or Campus Administrator will conduct whatever inquiries and/or investigations they deem necessary and appropriate to determine whether the student’s concern(s) are substantiated in whole or in part. Those inquiries may involve further discussion(s) with the student either individually or with appropriate Academy personnel.

  4. All inquiries and/or investigations shall be completed, and a formal response shall be provided, within 15 business days following the receipt of the student’s written notice.  If it is determined that the student’s concerns are not substantiated, the institution will provide a written explanation of the decision and deny the complaint; or if it is determined that the student’s concerns are substantiated in whole or in part, the institution will propose a resolution.

GRADE APPEAL POLICY

Foundation Hair Academy provides an opportunity for students to appeal grades in a fair and equitable manner.

The policy applies to all Academy students who are currently enrolled or were enrolled 30 days prior to submitting a notice of grade appeal to his/her Instructor.

Procedure for Grade Appeal:

  1. If a student is not satisfied with a grade assessed on an assignment or test, the student must provide a written statement of their concerns to the Instructor who assessed the grade within 5 days of the grade assessment.  The statement should set out the reasons for disputing the grade and include copies of any relevant evidence the student wishes to submit and have considered.  Please note, grades will not be adjusted for compassionate reasons or unexcused absences.   

  2. The instructor will reconsider or remark the assignment/test based on the criteria of the assignment/test and will provide the student with a written determination, setting out the reasons that led to both the original grade and any re-marked grade, if applicable.  Instructors are to provide students with their determination within 3 business days of receipt of the student’s grade appeal statement.

  3. If the student is still not satisfied with the Instructor’s determination, the student should submit a formal written grade appeal, together with all relevant supporting documents to the Academic Programs Manager within 3 business days.

  4. Upon receipt of the written grade appeal, the Academic Programs Manager will meet with the student to review the original assignment/test and all relevant supporting documentation (including the written determination of the Instructor). Academic Programs Manager may also meet with the Instructor to conduct a re-marking of the assignment/test.

  5. The decision of the Academic Programs Manager will be provided to the student in writing within 3 business days of receipt of the written complaint. All decisions with respect to the grade appeal will be considered final.  

WITHDRAWAL POLICY

Foundation Hair Academy’s Withdrawal Policy is intended to provide guidance for students wishing to withdraw from a program of study at the Academy.

Procedure

  1. Any student wishing to withdraw from a program of study must submit a written notice of withdrawal (together with any supporting documentation, if applicable) to the Academic Programs Manager, in person, by email or by registered mail.  The notice of withdrawal must clearly state the date on which the student intends to withdraw.

  2. The Academy will follow its Tuition Refund Policy in calculating any refund due to a student who withdraws.

  3. Students withdrawing from study must return any related equipment or supporting materials that have not been paid for by the student immediately upon withdrawal from their program of study.

  4. Equipment and/or materials must be returned in original condition as issued.

  5. Failure to return all equipment and materials in their original condition will result in a deduction for reasonable replacement costs of the equipment or material from any refund payable to the student.  If no refund is due to the student, the institution will issue an invoice to the student for reasonable replacement costs of the equipment or materials or any unpaid tuition in accordance with the Tuition Refund Policy.

DISMISSAL

Foundation Hair Academy expects students to meet and adhere to the rules and regulations of the Academy and its Code of Conduct (as set out in the Academy’s Respectful-Fair Treatment Policy) while completing a program of study.  Failure to adhere to the Academy’s rules, regulations, and/or code of conduct may result in dismissal or disciplinary actions.

Procedure for dismissal or disciplinary measures: 

  1. All concerns relating to a student’s conduct/misconduct shall be directed in writing to the Academic Programs Manager. Concerns may be brought by staff, students, instructors, or the public.  

  2. The Academic Programs Manager will arrange to meet with the student to discuss the complaint within 5 business days of receiving notice of the complaint.  If the alleged conduct is of such a serious nature that an immediate dismissal may be warranted, the Academic Programs Manager will meet with the student as soon as possible.

  3. Following the meeting with the student, the Academic Programs Manager will conduct any further investigations deemed necessary to determine whether the complaint is substantiated, in whole or in part.

  4. Any necessary inquiries or investigations shall be completed within 5 business days of the initial meeting with the student.

  5. Upon completion of all inquiries and investigations, the Academic Programs Manager will provide the student with a written determination with respect to the complaint.  In the event a student wishes to dispute a written determination; the dispute shall be resolved in accordance with the Academy's Dispute Resolution Policy.

  6. In the event the complaint has been substantiated, the Academic Programs Manager may:

    1. Give the student a written warning, a copy of which will be signed and acknowledged by the student and placed on the student’s file.  

    2. Set a probationary period with conditions that must be fulfilled or demonstrated.  During a probationary period, a student’s conduct will be monitored by the Academic Programs Manager.  Any notice of a probationary period will be signed and acknowledged by the student and placed on the student’s file.

    3. Dismiss the student from the program of study.  In the event of a dismissal, the Academic Programs Manager will provide the student with a written notice of dismissal which will include a calculation of any refund which may be due under the Tuition Refund Policy.  Any refund deemed to be owing under the Tuition Refund Policy shall be delivered to the student within 30 days of the date of the notice of dismissal.  In the event the student owes tuition or other fees to the Academy at the time of the dismissal, the Academic Programs Manager may undertake collection of the amount owing, forthwith upon dismissal.

    4. Request immediate payment or compensation in the event of any physical damage caused by the student to the Academy’s equipment or facility.

LANGUAGE PROFICIENCY POLICY

Instruction at Foundation Hair Academy is conducted in English. All prospective students whose first language is not English are required to demonstrate they have the appropriate English language skills to successfully complete their program of study.  

Procedure:

  1. Prospective Students whose first language is not English must provide proof of completion of a secondary school education English program at a grade 11 level or a post-secondary education program in English, in Canada or abroad.

USE AND DISCLOSURE OF PERSONAL INFORMATION POLICY

Foundation Hair Academy collects, uses, retains, and discloses information in accordance with the Personal Information Protection Act (“PIPA”). The Academy may share and disclose personal information within the institution to carry out its mandate and operations. Information, in aggregate form only, may also be used for research purposes and statistics.  

The Academy may also disclose a student’s personal information under the following circumstances:

  1. The College obtaining written consent from the student;

  2. The College is required to disclose personal information by law;

  3. The College is required to share personal information with Citizenship and Immigration Canada, as necessary for the purposes of an International Student program and in accordance with applicable provincial privacy legislation. 

  4. Student files are maintained in accordance with the College’s Records and Information Management Policy and procedures.

SAFETY POLICY

Foundation Hair Academy is committed to providing a safe and healthy working and learning environment for all staff and students. 

Procedure for Fire Safety:

  1. The Academic Programs Manager is responsible for ensuring adequate fire suppression equipment is available as needed throughout the campus and that it is inspected by a qualified inspector at least annually. 

  2. The Academic Programs Manager is responsible for ensuring all employees receive safety training with respect to fire suppression and fire evacuation procedures during their orientation.

  3. The Academic Programs Manager is responsible for preparing and posting emergency exit plans in each classroom at the campus with the exit from that room specifically highlighted.

  4. In the event of a fire emergency, the Academic Programs Manager, or a designated staff member, will dial 911 and advise the fire department of the location of the school.  They will provide details of the type of fire (if known) and the location of the fire within the campus.

  5. In the event of a fire emergency, all staff and students are to evacuate the campus.  

  6. Instructors will escort their students to a designated meeting location.  At the designated meeting location, the Instructor will check the students present against the list of students in attendance that day and will immediately advise the Academic Programs Manager if anyone is missing.

  7. The Academic Programs Manager or designate, will act as liaison between fire officials and students/employees during the emergency.  If necessary, the Campus Administrator will authorize school closure. 

  8. No student or employee will re-enter the campus until the fire officials have authorized re-entry.

  9. Students will be advised of all safety procedures during their first day of class and all emergency procedures will be posted throughout the campus.

Procedure for Earthquake Safety:

  1. The Academic Programs Manager is responsible for ensuring adequate precautions are taken throughout the campus to ensure that injury due to falling or unstable items during an earthquake is limited.  This may include securing file cabinets to walls and providing lipped shelving for books or binders that are located at or above head-level.

  2. The Academic Programs Manager is responsible for ensuring all employees review earthquake and evacuation procedures during their orientation to the Academy.

  3. The Academic Programs Manager is responsible for preparing and posting emergency instructions and emergency evacuation plans in each classroom at the campus with the exit from that room specifically highlighted.

  4. In the event of an earthquake emergency, all employees and students will take cover and remain under cover until the shaking stops.

  5. When it is deemed safe to do so, all employees and students are to evacuate the campus.  

  6. Instructors will escort their students to a designated meeting location ensuring that he or she takes the class list with them.  At the designated meeting location, the Instructor will check the students present against the list of students in attendance that day and will immediately advise the Academic Programs Manager if anyone is missing.

  7. The Academic Programs Manager or designate, will act as the liaison between rescue officials and students/employees during the emergency.  If necessary, the school will authorize school closure. 

  8. No student or employee will re-enter the campus until the rescue officials have authorized re-entry.

  9. The Academic Programs Manager will provide such further safety policies and procedures as are deemed necessary for program-specific safety issues.

PERSONAL SAFETY 

If you have concerns about your personal safety while attending classes, or if you notice any areas of the academy that you feel are unsafe, please contact the Academic Programs Manager.

RESPECTFUL & FAIR TREATMENT POLICY

Foundation Hair Academy is committed to ensuring that its learning environment promotes the respectful and fair treatment of all students.

While on the Academy’s premises or in the course of activities or events hosted by the school, Foundation Hair Academy expects students to meet and adhere to a code of conduct while completing a program of study. The Academy is a community of students and staff who are involved in learning, teaching, and other activities. In accordance with the code of conduct, all staff and students are expected to conduct themselves in a manner that promotes a positive learning environment. Students are expected to follow the Academy’s code of conduct (as set out in this Handbook) and are responsible for reading and understanding the Code.  Failure to fulfill these responsibilities may result in dismissal or disciplinary actions.

Students who do not meet the expected code of conduct will be subjected to the procedures outlined below which may include immediate dismissal from the institution depending on the severity of the misconduct.

Disciplinary measures:

  1. All concerns relating to a student’s conduct/misconduct shall be directed in writing to the Academic Programs Manager.  Concerns may be brought by staff, students, instructors, or the public.  

  2. The Academic Programs Manager will arrange to meet with the student to discuss the complaint within 5 business days of receiving notice of the complaint.  If the alleged conduct is of such a serious nature that an immediate dismissal may be warranted, the Academic Programs Manager will meet with the student as soon as possible.

  3. Following the meeting with the student, the Academic Programs Manager will conduct any further investigations deemed necessary to determine whether the complaint is substantiated, in whole or in part.

  4. Any necessary inquiries or investigations shall be completed within 5 business days of the initial meeting with the student.

  5. Upon completion of all inquiries and investigations, the Academic Programs Manager will provide the student with a written determination with respect to the complaint.  In the event a student wishes to dispute a written determination; the dispute shall be resolved in accordance with the Academy's Dispute Resolution Policy.

  6. In the event the complaint has been substantiated, the Academic Programs Manager may:

    1. Give the student a written warning, a copy of which will be signed and acknowledged by the student and placed on the student’s file.  

    2. Set a probationary period with conditions which must be fulfilled or demonstrated.  During a probationary period, a student’s conduct will be monitored by the Academic Programs Manager. Any notice of a probationary period will be signed and acknowledged by the student and placed on the student’s file.

    3. Dismiss the student from the program of study.  In the event of a dismissal, the Academic Programs Manager will provide the student with a written notice of dismissal which will include a calculation of any refund which may be due under the Tuition Refund Policy.  Any refund deemed to be owing under the Tuition Refund Policy shall be delivered to the student within 30 days of the date of the notice of dismissal.  In the event the student owes tuition or other fees to the Academy at the time of the dismissal, the Academic Programs Manager may undertake collection of the amount owing, forthwith upon dismissal.

    4. Request immediate payment or compensation in the event of any physical damage caused by the student to the Academy’s equipment or facility.

DEFINITIONS:

  • Discrimination:  Discrimination is conduct of any nature that denies equal privileges or treatment to an individual because of the individual’s age, ancestry, colour, disability or handicap, national origin, race, religious creed, sex, sexual orientation, gender identity, or veteran status.

  • Harassment: Harassment includes any unwelcome verbal or physical behaviour that unreasonably interferes with work/learning or creates an intimidating, hostile or offensive work/learning environment. General harassment can include, but is not limited to, remarks, jokes, or actions that demean or humiliate another person and deny individuals their dignity and respect. General harassment can also include bullying.

  • Violence:  Violence means an incident where the attempted or actual conduct of a person causes members of the learning community to be physically or mentally harmed, abused, threatened, intimidated or assaulted. Violence may take many forms, and it may be expressed verbally; through written or electronic communications such as email, internet, telephone, and standard social media channels; and/or through actual or threatening physical contact or gestures.

GRADUATION POLICY

Procedure:

Once all the noted requirements below have been met, the Academy will process the student’s transcripts and diploma within 30 days.

  1. Students must:

    1. Achieve a passing mark on each course within the program of study as set out in the Program and Course Outlines;

    2. Have paid all fees and tuition owing under the Enrolment Contract.

    3. Have returned all items borrowed from the Academy.

    4. Have paid all outstanding fines for items borrowed from the Academy.

    5. Provide the Academy with updated contact information in order that the Academy may contact them after graduation.
